Reservation Form

Bottle Service: For bottle service, a credit card is required to be on file. Please fill out the information below and the booth with bottle service under the Given Name will be reserved for you.

Food & Drink: Credit card information is taken to make a reservation for food and drink reservations. Your credit card will be on file in case of no-shows. If you do not show, or cancel in less than 24 hours in advance you will be charged. No-show/late cancellation fee is $50.
